At Care Network of Alabama's Health Home, our goal is to help Medicaid recipients with certain chronic health problems stay as healthy as possible. Our specially trained teams look for opportunities to keep you well. Your nurse or care coordinator will guide you on your path to improving your health.

Qualified nurses, care coordinators, doctors, pharmacists, and others will work together to help you achieve your best health. We provide services at the doctor's office, in the hospital, at your home, or by phone.

Qualifying Health Conditions

If you are a Medicaid recipient and have one or more of the conditions below, you may qualify for help with your health care.

Our Care Coordinators Can Help

Our care coordinators want to help you live a healthier life. We will work with you and your doctors to help you better understand your health.

We will help you schedule your doctors' visits and can even help get you a ride to your medical appointments. You can count on us to answer your health questions, help you manage your medications, and connect you to the community resources you need.


Transitional Care

If you have to spend time in the hospital, our transitional nurses can come to your home after you're discharged. We will help you better understand your medicine and health conditions.

Prescriptions

Our pharmacists can help you make sense of your medications. We will look over your medications and help answer your questions.

Behavioral Health

Our behavioral health nurses can help you with any mental health or substance abuse problems. We can attend your doctor visits and help you make follow-up visits.

Are You a Patient 1st Provider
Who Wants to Join?


There is no fee for providers to join or participate in the health home. In fact, some providers may be eligible for enhanced case management payments each month. Patients are assigned to the health home based on the provider's Patient 1st panel.

Want to Make a Patient Referral
to Our Health Home?


Anyone can refer a Medicaid recipient to this program. Patients may be referred because they are in the hospital, because their doctor suggests it, or patients can even refer themselves. There is no charge for this program.
